CliniTouch Vie is a software with a personal service and support levels can be flexed as needed by our clients.
Clients have access to an online/real-time reporting tool which is triaged by the development team. Clients receive receipt of issues and confirmation of any action taken.
Clinicians have a designated operational manager who can access any level of support for the client which can include collaboration with the development team if required.
Clinicians have access to the CliniTouch Vie Product Owner via email/Teams/phone and in ‘normal’ times face-to-face. CliniTouch Vie Product Owner is also a clinician which offers an extra level of support for Clinicians and patients alike.
Cloud support is provided by Spirit’s own dedicated IT Team in partnership with a dedicated Microsoft Azure support engineer. Spirit’s internal IT Team are on site and are monitoring the platform continuously.
Training support is provided using static and video content. This can also be delivered at a team or individual level where required using screen sharing and Q&A sessions. In ‘normal’ times face to face support is available. User guides are also supplied for clinical and end user process confirmation and future guidance. - Support available to third parties

Responsibility for helping users to start using our service will be taken by our Programme Manager. Our range of resources will be adapted for local use. We will establish a user forum to test and develop materials as part of the mobilisation process.
The on-boarding package will comprise:
• For individuals receiving a CliniTouch Vie installation through Spirit, training will be provided onsite at the time of the installation
• For all system users and system super users commencing one month prior to go live date on-site training will be delivered on the clinical and patients’ systems and associated interfaces. This training will be delivered at local venues for staff
• Ongoing sessions delivered on-site and locally as required throughout the duration of the contract for new starters and for existing users to explain system updates and new functionality
• Our written user manuals for users, and super users are in plain English and are approved through our medical approval process and through user accepting panels. We have significant experience of importing and exporting patient data to and from clinical systems and has robust processes and procedures in place for when the contract ends. We adhere to guiding principles laid out in the NHS Digital Exit and Transition Process 2015, Confidentiality; NHS Code of Practice 2003, The Data Protection Act 1998, Caldicott and Caldicott 2 and DH Records Management Code of Practice.
We will implement a secure method of data extraction and:

The user interface in CliniTouch was designed to be very clear and convey any information to the user in simple language. The system was designed in collaboration with a user interface specialist, so we could ensure that screens were made with a high readability level, considering text and background colours, text size (whether at normal size or zoomed in by 400%), not using images of text, ensuring that the design is responsive.
We have not done any specific testing with users of assistive technology to meet accessibility standards. However, we ensure that every effort has been made to caters for a wide range of accessibility requirements and continues to review on a regular basis with our clinician and user forum. Due to Android’s innovative accessibility options, we can cater for hard of sight with increased contrast or larger text, or text to speech. Spirit’s CliniTouch Vie service is hosted on Azure servers, which are all located in the UK with a 24-hour per day response time provided every day.
We are able to set up capacity warning systems via Applications Insights in Azure to alert of any critical server capacity issues. We mitigate against reaching any server limits by having full capability to rapidly flex our server capacity as needed in response to requests. Azure SQL Server capacity can be increased instantaneously, although alerts are in place to monitor capacity allowing us to increase space or performance before they become critical.

MS Azure have ensured that their servers meet the latest NHS Digital standards, utilise the latest anti-virus and meet industry-standards for information governance.
Up-time is constantly monitored and all customers are notified of any significant downtime with an SLA in place to resolve.
Servers are hosted by MS Azure who offer 99.9% up-time and have failover servers located in a different city, if required.
Our architectural pattern includes managed rulesets on managed gateways provided by Azure. We have a DDOS and CDN mitigation layer.
We have protective monitoring in place linked to our internal support teams along with an audit service for traceability.
On top of this we can gracefully fail at the point of ingress to the platform.

Due to patient data regulations, patients cannot be contacted by Spirit when there is a network outage as they would not have access to their contact details. Outages are communicated by email to all clinical service users and admin(s), as well as by telephone to all clinical leads. All clinical users are then kept up to date, where appropriate. Local arrangements are put in place for clinical users to contact the patients directly.
If the issue is due to cyber threat, we will consider keeping service offline until safe and communicating this with the relevant users.

We operate an ISO 9001 Certified Quality management system of which our ISO27001 compliant Information Security Policy (a copy of which is available upon request) is a part.
The cloud environment used within Spirit Digital is run over Microsoft’s Azure cloud network. All data (including backups) will be held in Spirit’s Microsoft Azure system over encrypted SQL databases within the UK.
PID data that is transmitted over the internet is encrypted and is received by the Azure hosted servers, where the data is analysed and stored within the UK. The two URLs used; for clinician and patient access, are both SSL encrypted, enabling encrypted data transmission via TLS 1.2.
As the systems are run over Azure, we can be rest assured that they comply with ISO27001 standards and are a compliant data centre to store all NHS data.

The software is provided as a link to the patients, or already pre-loaded into a tablet or smartphone where we provide the device. Fully user-acceptance tested updates are automatically downloaded to the devices, or updated on the site, without any requirement for patient intervention or action.
The clinician portal is accessed from NHS managed IT equipment and the data privacy and security for clinicians is administered locally by NHS Health Informatics Services.

CliniTouch Vie enables clinicians to manage more patients remotely than they could have done face to face, which means, especially during the COVID-19 pandemic, that scarce clinician resource can be more effectively deployed to care for those patients who need it most, at the most appropriate time.
This efficiency maximises the benefit for the community in terms of less pounds spent per cohort but also means a reduction in number of home visits required (a 47% reduction in two of our most recent pathway deployments) which helps reduce carbon emissions from clinician travel.
As an example, Spirit and Leicestershire Partnership NHS Trust worked together to deploy CliniTouch Vie into two pathways of care – COPD and Heart Failure. The remote management of patients means that community nursing teams can monitor, message and video call patients thus reducing the need for home visits.
In the most recent study, the 13-week adjusted average number of home visits dropped from 2133 to 1121 (a reduction of 45%). With an average home visit cost of £91, this represented a quarterly saving of £92,000.
The average visit distance travelled (to and from base) is 14 miles, representing a reduction in miles travelled of 14,168. Assuming a small size average petrol car, this equates to 3.3 tonnes of CO2 per quarter saved, or 13.2 annually.
This saving will be enhanced by the further introduction of virtual Pulmonary Rehabilitation on the CliniTouch Vie platform. For each 100 patients using the platform rather than visiting a community centre 6+ times for their exercise programme, a further 1.85 tonnes of CO2 emissions may be saved. - Covid-19 recovery

We provide a virtual ward for patients recovering from COVID-19, enabling high risk and post-discharge patients to continue their recovery at home, supported by their specialist clinical team.
Our COVID-19 Virtual Ward has been deployed effectively by Leicestershire Partnership NHS Trust and University of Hospitals Leicester Trust to help support caseloads, manage admissions and prepare for winter pressures.
One of the major benefits of using a virtual ward is expediting the rapid discharge of patients to help free up beds. At a time when hospital occupancy rates are climbing and with the usual winter pressures still to come, enabling earlier discharges without compromising patient safety is crucial to the ongoing recovery.
The virtual ward was found to be successful in assisting with earlier discharges and reducing clinically necessary re-admissions for patients admitted with COVID-19 – without compromising on patient safety.
The evaluation study, in collaboration with Leicestershire Partnership Trust and De Montfort University, looked at the clinical and economic outcomes of a digital pathway, allowing patients to be discharged from acute hospital care and monitored within a community setting by a specialised team. Our early evaluation study of 65 patients found that the COVID-19 Virtual Ward delivered a significant 40.3% reduction in the average length of hospital stay for COVID-positive patients, from 5.5 to 3.3 days.

Patients with long-term conditions may be impacted greatly from a health & social perspective. They may struggle to find work and then maintain their role due to the impact of their condition due to increased absence to manage their health and well-being. This may impact their family/carers who have to provide additional support and therefore impact upon their employment status.
This disparity in income can impact other aspects of their life such as access to information/technology that would enable them to be empowered, as well as the financial burden that travelling to/from appointments can bring.
Areas of focus for Spirit include:
• Public empowerment: Technology gives users the opportunity to have their own health promotion companion, which the user owns/controls, reducing the burden on health care resources. One of the key benefits of CliniTouch Vie is that is accessible via a range of hardware, including Mobile Phones, Tablets and Computers, offering bring-your-own device functionality; but we can also work with providers, subject to agreed costs, to provide devices.
• Social engagement/inclusion: Patients on benefits or low incomes can find it difficult to meet the cost of travelling to healthcare premises. This can widen health inequalities. The service has the potential to level-up healthcare and enable people who are often excluded to benefit from and receive quality health services. Via offering digital solutions patients can interact remotely, providing their data direct to their clinician and better manage their condition.

CliniTouch Vie can receives data from various peripherals, manually or via Bluetooth. Data is collected from these peripherals and combined with softer signs (from question sets) to give a patient risk score using CliniTouch Vie proprietary intelligent algorithms. CliniTouch Vie has a dedicated real-time clinical dashboard where authorised users monitor and triage patients, view vital signs measurements, observations, view alerts and early warnings (e.g. NEWS2/RESTORE2), receive reminders and clinical messages.
RAG Rating: For triaging, all biometric and soft-sign data collected is analysed and coded to generate a ‘red-amber-green’ (RAG) rating to alert staff to deterioration, which appears in a real-time dashboard available to clinicians and care home staff. The RAG rating ascribed by the system enables any interventions to be made in a prioritised and timely manner
Clinician Dashboard: Vital signs measurements for individual patients are easily accessed and laid out in a single page facilitating rapid review. Our dedicated clinician’s portal enables clinicians to view the vital signs data of a particular patient’s readings over time, in an easy to visualise way that trends changes over time and actively identifies and flags any deterioration. CliniTouch Vie securely collects, stores and presents data to facilitate easy clinician access to the recorded vital signs via the clinician portal, supporting them to make timely decisions about patient care according to individual needs.
