The highly configurable Dermicus platform allows organisations to augment their existing clinical systems with teledermatology capabilities. Dermicus can extend the functionality of a primary care EPR provide multiple pathways (skin cancer, wound, general photo, or clinical communications) that automate data flow between Dermicus and a core clinical system.

Onboarding and offboarding

Getting started
Gnosco has designed the Dermicus solution to be extremely simple to implement and start being used by clinicians. Our solution is specifically designed to support organisations to start using Dermicus with none or little implementation support, allowing fast and easy onboarding.

Once the commercial arrangement is agreed, the Dermicus app will be installed on the required end user devices and user names and logins for the platform allocated. This can be done remotely by the Dermicus team.

All that is required after this is to log in and begin using the solution. When you log in and open a new request the users will connect automatically with the clinical centre they are assigned to on the Dermicus Platform. There's no special action to make this happen and levels of access can be allocated per role of the clinician.

Dermicus recommend onsite training in how to use the solution; however, the app has been designed to intuitively lead users along pre-defined pathways.

End-of-contract data extraction
At the end of the contract, Gnosco aim for offboarding and data access to be as simple as possible. Gnosco will make all data input into the solution freely available to the buyer. It shall be the buyer's responsibility to engage the services of a data transition expert or a replacement service. Gnosco will provide a full data extract which shall be provided either to the buyer's replacement service or data transition expert. For the avoidance of doubt the Buyer shall be responsible for the migration of data into their new solution.
End-of-contract process
Gnosco have designed the service to have as simple an exit as possible. From an end user device perspective Dermicus is like any other iPhone app and can be simply removed from the device. No data is stored on the device so once the app is deleted from the device it can be used as normal and will not store any Dermicus data.

The Gnosco customer team will be in contact throughout the term of the agreement and will support any data migration requirements required.

The mobile App must be installed to ensure the secure transmission of patient data into the Dermicus platform.

Secure access to the web App is provided by two-factor authentication by SMS code or smart card access. The Web App does not require any installation.

At present the Dermicus solution has not specifically designed with any formal accessibility features. However, the solution has been designed to be easily navigated and clearly laid out to aid comprehension and support safe clinical practice. Should the customer have any specific accessibility requirements please discuss them with the customer team.
Please note that Gnosco have completed DTAC and comply with DCB0129 which takes into account the clinical safety of our solution. Our development methodology takes useability and accessibility into account to ensure clinical safety. Gnosco works closely with its customers to ensure the solution meets clinical safety requirements.

Description of customisation
Dermicus has an external API which can be used to build integrations. Next to this, we can build custom integrations via an internal API, ensuring Gnosco can assure our customers of the clinical safety of any integration that is undertaken. This approach also ensures that customers are not limited by the current capabilities of the external API.
Dermicus can be customised in several ways; for new patient pathways and practice administration processes, customer specific forms and content, customer specific organisation/centre/user setup, notifications, roles, different products within Dermicus. Some of those above can be configured by users, some by centre admin, some by customer admin and some by Gnosco admin.

Fighting climate change

One of the core principles of the Dermicus platform is to use technology to link up clinicians and patients that are in different geographic locations. For many patients, particularly those in rural areas, accessing specialist dermatology services mean travelling long distances to a centre of care. This has an inherent impact on the carbon cost for the delivery of care. Dermicus dramatically reduces the need for long distance travel and provides all the services needed to provide remote care, advice, and guidance.
Not only does Dermicus fight climate change by reducing the need of patients and clinicians to travel but it also reduces the overall time to treat. This reduction on NHS services has an inherent impact on the carbon cost of the NHS for dermatology as a whole. The longer clinicians take to assess, review, and undertake a body of care the greater the impact that service has on the consumption of energy within the NHS.
Our implementation services also look to provide insight to NHS Staff on how to structure their services to drive efficiency and consideration for the environmental impact of how they provide services. This may include setting up specific remote care time which allows clinicians to improve the efficiency with which they can complete Dermicus related tasks.
We continually look to develop further features that either reduce the amount of time a patient requires care or provide additional services that cut down the need to travel to continue driving Dermicus’ ability to mitigate climate change.

Equal opportunity

The Dermicus platform dramatically increases the equality of opportunity both for clinicians and patients. Specialist dermatology services are not available in all relevant settings and tend to be found in urban or high-density areas. This leave clinicians in rural areas unable to access the same key functions of their urban peers. Dermicus removes the geographic barrier to care by permitting remote consultation and collaboration with centres of excellence. This is a vital step in mitigation healthcare inequality and minimising the workplace inequality between NHS staff in remote areas.
This has a knock-on impact for patients, particularly those in rural areas, who can benefit from access to a wider array of services and, in turn, have access to the outcomes and quality of care available in other parts of society.

Wellbeing

The main aim of the products and services provided by Gnosco is to improve the wellbeing of our customers and, by extension, their patients. We develop tools offering new ways of working that are more efficient, provide swifter care of patients, and reduce the workload on NHS resources.
These tools have the ability to dramatically improve the wellbeing of NHS staff who can utilise their time more effectively as well as freeing up time to spend on other patients.
Clinicians in rural areas, in general, have fewer resources and lack the access to tools and knowledge in better served areas. This has an impact on the wellbeing of clinicians in these areas, who are faced with the same targets and desires to provide high quality care to their patients. The Dermicus platform is designed to “level the playing field” for these clinicians by providing them access to specialist consultation that would otherwise be impossible. Enabling clinicians to provide better outcomes and provide them with the same tools as better served peers is of undeniable importance in mitigating healthcare inequality and improving the wellbeing of NHS staff.
For patients the increase in wellbeing is immeasurable, particularly where Dermicus is being used to assess cancer related conditions. Some patients face long referrals and waiting times for basic results and confirmation of the status of any skin abnormality. This waiting time is stressful and leaves patients fearing a cancer diagnosis, which in the majority of cases is not an especially difficult or lengthy conclusion to reach – the delay is purely administrative. By providing tools to clinicians that improve their access to specialist knowledge and support the wellbeing of patients waiting for diagnoses can be significantly improved. Beyond this there is an obvious benefit of patients achieving an earlier diagnosis have better outcomes
