Severity definitions: Critical = total service outage, security or data incident, or patient-safety risk; Major = severely degraded service affecting multiple users with no workaround; Standard/Minor = single-user issues, non-urgent defects, configuration or how-to requests.

Response SLAs: Critical 60 minutes; Major 3 hours; Standard incidents 24 hours; Service requests 48 hours. Escalation: Service Desk -> Duty Manager -> CTO. We give status updates until resolution and a post-incident report for Critical incidents.

Following the principles of Kaizen, we strive to continuously improve the utility and usability of our solution through user feedback received from various deployments with NHS ICBs and Trusts, Local Authorities and Private Care Providers. Additionally, in an Innovate UK sponsored project, we have co-developed a bespoke solution for Discharge to Assess (D2A) based on stakeholder requirements captured from LLR ICB, UHL and LPT Trust, Social Care Team and older adults (ex-D2A patients).

What users can and can't do using the API
Setup and access: Our REST API is documented and versioned to HL7 FHIR R4 and UK Core profiles. Access is by vetted organisations only, with application registration and production credentials issued after IG checks and a DPA. Authentication uses OAuth 2.0 with JWT; all endpoints require HTTPS/TLS 1.2+.

What users can do: Create and manage organisations, sites, users and device records; subscribe to webhooks; read time-series and event data using FHIR resources such as Patient, Observation, Device and Encounter; write permitted updates via POST, PUT or PATCH; retrieve audit events. Field-level filtering, pagination and replay cursors are supported.

What users cannot do / limits: No schema changes or direct database access. No bulk deletes. High-risk writes are queued and audited. Rate limits apply. Only least-privilege scopes are granted and some PII elements are redacted unless the buyer’s lawful basis and access model are confirmed. We may refuse or revoke access where security, privacy or safety risks are identified.

Change management: Backwards-compatible changes follow GDS API standards, with deprecation windows and release notes in the developer portal.

We have two standard kits:

1) MiiCare Wellbeing package with 12 months subscription, including MiiCare kit (MiiCube device, smart watch, 1 room multi-sensor, 3 room mini sensors, 1 door sensor), up to 5 remote monitoring user licenses (access to web application, dashboard and mobile app), including Monica content and services.

2) MiiCare Longevity package with 12 months subscription, including MiiCare kit (MiiCube device, smart watch, 1 room multi-sensor, 4 room mini sensors, 2 door sensor, medication box sensor, BpM, thermometer, pulse oximeter), up to 5 remote monitoring user licenses (access to web application, dashboard and mobile app), including Monica content and services.

Add-ons include - additional room sensors, additional door sensors (for back door and appliances that have a door, e.g., fridge), smart plugs (for appliances like TV, coffee machine or kettle), digital thermometer, digital oximeter, digital blood pressure monitor, digital weighing scale, visitor tokens, and medicine sensor.

Capacity is managed using autoscaling and load balancing, with queued processing for peak events and rate limiting where required. Our system's uptime is >99.8%. SLAs are in place with our clients. We notify our users of any planned maintainance related system disruptions beforehand.
Approach to resilience
MiiCare runs on Microsoft Azure UK regions with availability zones, spreading workloads across separate datacentres with independent power, cooling and networking. Multi-zone deployments support high-availability targets up to 99.99%. Data uses geo-redundant storage with designed durability of 16 nines, with backups replicated to the paired region. We monitor via Azure Service Health for alerts and incident notifications.

Any released patches relevant to the software and services provided by MiiCare are reviewed. Any critical / high severity vulnerability are applied within 14 days of release. Vulnerabilities discovered in the tests are patched and fixed, which is followed by another round of pen tests to ensure that the discovered vulnerabilities are addressed and patched.
